## Summary
- Replace Cognito and Identity Center stubs in team-provisioner Lambda
with full implementations
- Cognito sync: creates `team-{name}` group in internal pool,
adds/removes members by email lookup
- Identity Center sync: creates `team-{name}` group in identity store,
adds/removes members by email
- Adds IAM permissions for `cognito-idp` and `identitystore` APIs to
Lambda role
- Wires Cognito pool ID and Identity Store ID from modules to Lambda env
vars
## Test plan
- [ ] CI plan passes with new IAM statements and env vars
- [ ] After merge+apply, invoke Lambda with platform-test-team to verify
