simplify jdbc module

• supports jdbc url at construction time. see jdbc settings programmatically #429
• get rid of lazy initialization, it complicates bootstrapping, makes submodules hard to implement

Sub/dependent modules will/must be updated too, but we don't expect any breaking change on existing users. Sub-modules includes: jdbi, hbm, ebean, etc...
