Replaces the old rate_limiter with a new limiter module using token bucket and connection guard for improved DoS protection. Refactors authentication handlers to use unified AuthSettings and removes redundant AuthConfig. Restructures kalamdb-auth crate into submodules for authorization, helpers, models, providers, repository, security, and services, with many files moved or renamed for clarity. Updates dependencies (e.g., thiserror, colored, cc), and switches sqlparser version. Cleans up Notes.md and updates usage across API handlers and tests.
