docs: align README example with actual markdown output

PROBLEM: README shows a textual summary block, while current markdown output is table-only (and metrics currently incorrect per related bug).

SOLUTION:

• Either enhance markdown reporter to include summary header or update README example
• Add a verified example file and a test to prevent drift
