Add a flattened Claims::extra map so typed CWT claims retain application
and private claims during decode and encode round trips. Update docs and
examples for the new behavior, switch crate metadata/docs to MIT-only
licensing, and remove the redundant CLAUDE.md shim in favor of AGENTS.md.
