Appendixes Section and Subsection Pages are Duplicated

What steps will reproduce the problem?
1. Go to Appendixes
2. Then PostgreSQL Error Codes
3. Then Date/Time Support
4. And notice that their are duplicates both w2ithin each subsection and of all 
the Appendixes further down in the Appendixes Section

PostgreSQL 9.1.2
Windows 8 x64 Professional

Must be a bug in the DocBook Stylesheets/Code for .CHM generation.
