Fixed

• lex-lsp: include-not-found diagnostic now points at the offending :: lex.include src=… :: annotation instead of falling back to the document head. Without this fix, vscode rendered the diagnostic as a zero-length point at line 1 col 1, giving no signal which include in a multi-include doc was broken. IncludeError::NotFound now carries include_site: Range; the resolver wires annotation.location through. The other no-site error variants (RootEscape, LoaderIo, ParseFailed) still fall back to head_range for now — same fix pattern applies but kept out of scope here. (#500)
