from_caliper Examples Not Working for Caliper>=2.10

Node ordering breaks the from_caliper examples shown in https://llnl-hatchet.readthedocs.io/en/latest/analysis_examples.html#read-in-a-caliper-cali-file, with the error No hierarchy column in input file. This is also documented in #123. We can recommend users to use the from_caliperreader (#160) reader instead, which does not have this issue.
