Describe the bug
Since upgrading to Loop v3.12.1 (57) all my pods are crashing with a critical pod fault after two days of functioning correctly. The pod faulted while it was in the middle of delivering an immediate bolus.

Steps to reproduce the behavior:

1. Use the pump normally for 2 days... get the critical error.

Expected behavior
Don't have a critical error.

Not sure if anyone else is getting this error, I don't think it's a bad pod lot as it happened across two different boxes.

This is a known issued and is POD lot related, not loop. Please check the docs for more information and close this issue. Ben

Might be POD battery related, nothing loop can actually do...

Same. Tried upgrading to iOS 26.5 to no avail. with every 56-72 hours I have left on a pod , it craps out. idk why. same thing has happened last 4 pods, going back to the dash handheld now after a year and a half.

I’m not trying to be rude or unkind but if you're unwilling to research or keep up with ongoing news in the loop world you should not be using the software. 203 critical pod errors have been widely discussed over the past 6+ months in the community. If you haven't checked in to the community in the past 6 months, you really shouldn't be using DIY loop. The current community supported discussion points to likely a marginal batch of batteries being installed into pods, or a shift in power required to manage the pod due to the change from SAW to ATLAS Bluetooth boards (or both). Neither has anything to do with loop, nor can the developers make any alteration to fix this on going issue. The hope is that whatever changed at a manufacturing level was a temporary issue or will be fixed by Omni-pod over time via revision. Please check in and follow along if you plan to keep using DIY loop regularly. Ben

There is no disclosure from Omnipod officials, neither any discussion on this issue outside the looping community, is it possible to identify affected LOTs?

This recall is for a different known issue. There was some tracking early on with the 203 errors, someone else may have the link. But it was hard to properly track since the same lots didn’t consistently fail. We traveled to France back in April, had 2x 203 failures and then have had nothing since. The lots the failed then are some of the same lots we continued to use. In total we’ve seen maybe 6-10 failures in the past 6 months due to 203 errors (less than many). Ben

Seems like every pod in my current batch has given me 203 with approx 20-30 hours left and almost always about 40-50U insulin.
Just put on a pod from another batch, hoping it will not give me a 203.

I'm just going to call Omnipod and let them know I'm looping and every pod in that full batch has given me 203 errors. Hopefully the next box won't give me as many issues. TBD

Also been having this issue very recently. Spoke with someone from Omnipod and they mentioned "improvements" done to the omnipod dash device which may contribute to critical pod failure on those using Loop app but there is nothing they can do about that. Same issue here, usually day 2-3 of use with more then enough ?50units of insulin remaining.
