Fix first line indent in block when inlined#7

When the first line of a prose block appears on the same line as the @mdexp directive, it is now excluded from the minimum indentation computation. This allows the remaining lines to be properly dedented based on their own indentation, while the inline first line is fully left-trimmed and emitted at indentation zero.
