I added a few suggestions for re-arranging this.... admittedly just to more closely align with the way that I've typically made PR's.
Approving anyway, because this is a vast improvement over the previous template (which I was pretty much always deleting wholesale), and I'm happy to try to standardize on a different format if we prefer.
