Excluded items cannot be discerned

Currently, both included and excluded microflows and pages are indexed in the catalog. There is no column indicating the document is excluded.
Suggestion is to add that column and make availabe CHANGE statements for documents to toggle excluded state.

Investigation

The Excluded boolean is already parsed from BSON for all document types (microflows, pages, workflows, enumerations, constants, etc.) but isn't surfaced anywhere — not in SHOW/LIST output, DESCRIBE output, or the catalog tables.

Plan

1. @excluded annotation — following the existing @Position, @caption, @color, @annotation pattern:

DESCRIBE will output @excluded for excluded documents and activities, making it roundtrip-safe.

2. Catalog — add Excluded BOOLEAN DEFAULT 0 column to microflows, pages, and other catalog tables.

3. LIST output — mark excluded items with [EXCLUDED] suffix or similar indicator.
