To Reproduce

1. Create some questions/models with sample db
2. Remove sample db
3. Restore sample db
4. Create new document
5. Type / and choose "Chart"
6. Type to find one of the removed questions
7. It's returned, click it

It's added to the doc, but it does not exist.
