Uploaded to the Python Package Index as "ctgen 0.2.0"

Very brief summary of changes:

- a few misc fixes and improvements
- use and adapt to v0.2.0 of the template-text Lua module
- tested in Windows
- large refactoring of both the CPP and Octave backends, with
  changes in the generated code too
