Which versions of SQL Server and which OS are affected by this issue? Did this work in previous versions of our procedures?

Can you please verify you have the proper permissions and are invoking powershell as an admin as enumerated here:
https://github.com/microsoft/DiagManager/wiki/Run-PSSDiag-to-collect-logs#prerequisites
