fixes #419

This PR:

  • Reverts removal of the visualizer from the standalone VSIX
  • Re-enables building and publishing the component VSIX (which can be integrated into the VS installer)
  • Disables auto evaluation of visualizations in the component VSIX, requiring a refresh click, per VS policy
