Tool validation fixes and improved MSAL error handling 🛠️

This release improves tool validation to ensure the complete registered tool set is correctly recognized at runtime. It also enhances MSAL error handling with richer, PII-safe telemetry and more accurate exception mapping, making authentication issues easier to diagnose.
