Id format changes: no need to support old format

We report parentId and Id in the format of |traceId.parentSpanId and |traceId.spanId.

This is not needed anymore even for backward compatibility reasons. Compatibility with old versions of Application Insights SDK is handled by Breeze.

• parentId should now match parentSpanId
• request/dependency.Id should match spanId
• link id should match linked context span id
