Symptoms:

• Portal: "Error loading graph: Failed to load system tools"
• GET /api/v1/extendedAgent/tools → 500 "INTERNAL_ERROR"
• PUT /api/v1/extendedAgent/apply (kind: ToolList) → 500
• Test Python Tool button works (code executes successfully)
• Create Tool button fails (cannot persist)
• GET /api/trace/health → 200 OK (agent healthy otherwise)
• EnableWorkspaceTools: true
• User has SRE Agent Administrator role
