Clarify scope of and when custom instructions are applied

There's confusion about the different scopes of instructions files (workspace vs user) and when they are actually applied (manually vs automatic). Clarify these topics in the Custom Instructions docs.

Also clearly describe that repo-level instructions are not supported (as is the case in GH.com).
