Crash when debugging Code-OSS, and start an extension debugging session

This scenario has been pretty unstable for a while, but lately it just basically never works.

Steps:

1. Start debugging VS Code from source
2. Open an extension folder to debug in the resulting VS Code window
3. Start debugging the extension
4. 🐛 Both the Extension Development Host window and the debugged VS Code window will crash (very often immediately)

OS: Windows 10

I just went through the steps and I'm unable to reproduce the issue. Does it still happen on the latest Insiders?
