Stopping an extension debug session doesn't close the Extension Development Host

I can reproduce. This seems to work with stable -> regression.
I can not reproduce when running out of source (so I can not bisect).
I see that the vscode properly sends out a disconnect to the adapter.

@weinand @connor4312 did you change something in this area while I was on vacation?

@isidorn No, I haven't touched anything in VS Code (other than the new split button).
But I was going to report the same issue because I was seeing this problem with Mock Debug every day 10 times (but not in this simple scenario).
