Notebook search: context menu sometimes isn't correct

Repro:

1. search.experimental.notebookSearch: true
2. Open a notebook
3. Search in the notebook using something that would have in-code and in-output results
4. Click on an output result
5. Right-click a code input result
6. Context menu should have option to replace

Seems to only happen sometimes...
