Put a debug configuration selector in the Run menu

Instead of making me type Ctrl-Alt-P and search for a item I may or may not remember the name of, please put a list of debug configurations in the Run menu. I also suggest adding the ability to auto-select a debug configuration based on the type of the current file. So if the file is a ps1 file, select a PowerShell configuration.

This is available in the Run and Debug panel:

If you are not seeing that in your instance of Code you can right-click on the gear icon and select to "Reset Menu" or "Start Debugging" to unhide it.
