Extensions - publisher trust dialog shown while creating a profile

Steps to Reproduce:

1. Install the latest VS Code Insiders release
2. Create a new profile from the Python template
3. During profile creation the "publisher trust" dialog is shown multiple times

Thanks for filling this one Lad. I just went through this experience, and I can reproduce.

I do like that the user gets asked, but I agree that multiple dialog are overkill.
We do curate these extensions and make sure that what we have in profile templates should be OK. So I am fine not showing any dialog. But ideally, there would be one dialog to inform the user about all.

Or the profile creation template, should at least have the Extensions node expanded - so the user actually sees by default what extensions are about to get installed. Right now it is collapsed by default. And ideally we render the extension authors next to extension names.

I will be showing the following one dialog (which is not perfect but better than multiple dialogs) as it is late in the endgame.
