Sign-in can get stuck from Chat

1. Sign-out from GH Copilot
2. Open chat view, ask model what is 2+2
3. Sign-in process starts, agree to open browser
4. Do not complete sign-in in browser, just close that tab
5. Go back to vscode. Stop the sign-in process via Stop action
6. Try to sign-in now. Impossible 🐛

I just killed the first progress notification to be able to click on the Stop button.

This is somewhat annoying to solve at the auth extension layer because:

1. dismissing the progress notification does not trigger cancellation on the Progress object (VS Code API)
2. the auth API that sign in uses does not have cancellation token support

I think the right thing to do here is probably allow createSession on an Auth Provider to take in a cancellation token... which would then cancel the flow and free things up.

The quick fix is probably having the Stop button ignore the previous sign in flow... If it's already doing that there might be code in the GH Auth extension that serializes sign ins, which makes some sense... we could have subsequent sign ins cancel any in-flight or something
