In my case this happen when Opus tries to create long report files.

The work around I found is to ask to pass the following prompt after a crash:

Write your all your outputs to files in chunks of no more than 1000 words each. Create it with the first chunk, then append subsequent chunks to the same file. After each chunk, stop and wait for me to say "continue" before writing the next chunk.

It works fine. Just annoying... :(
