Sessions: Add top-level entry point from VS Code

As discussed today, it'd be great to consider a top-level entry point(s) from VS Code into Sessions (it can use the existing command to open the sessions window).

Some ideas:

• A tip in the chat panel

• An entry point in the chat command center menu
