copilot --resume uses the old name of the session

Testing #312453

1. Start a new CLI session in VS Code Chat
2. Rename it in VS Code sessions view
3. Open integrated terminal, do copilot --resume
4. Notice the session name is the old one 🐛

Cant seem to repro:

Are you on latest copilot cli and latest insiders?
Also see if typing another request to copilot cli session after renaming makes the session show up.

Yes

I did "what is 2+2" in copilot terminal. And the cli session from Chat view does not show up.
