Previous AI suggested edits won't go away

Type: Bug

I don't know how to reproduce exactly. But something like:

1. Use AI Chat in Agent mode and have it edit a jupyter notebook.
2. (not sure here) While suggestions are there ("Keep" or "Undo" buttons exist), go away. Maybe let computer sleep, Maybe close and reopen, unsure.
3. I come back to the notebook and there are suggestions without the ability to click Keep or Undo anymore. They are grayed out.
4. You can close VS Code, close the document, do anything you like, but I cannot seem to get these suggestions to disappaer now.

Notice how the buttons are grayed out. I can't figure out how to get these old suggested edits from AI to go away.
