Disable Extension prompt 2 notifications

Steps to Reproduce:

1. search and install python extension (id:
   ms-python.python)
2. search and install black formatter extension (id:
   ms-python.black-formatter)
3. disable python extension, prompt black formatter depends on it.
4. click disable all button, prompt python debugger depends on it.

The reason why 2 notifications show up is that python is an extension pack which includes python debugger, but python debugger also depends on python. First notification ignores python debugger as a pack extension and only check other extensions whether depends on python. After clicking disable all button, it check dependencies again and python debugger depends on python and second notification shows up. I wonder why ignore pack extension for first check. In such case, if an extension is a pack extension but depends on parent extension. Two notifications show up and confuse users.
