Comment actions are poorly rendered if only title is provided for the command

Steps to Reproduce:

Add comment actions (comments/comment/actions) with the comment API.

Actions render clipped and overlapped.

You can use the comment sample extension https://github.com/microsoft/vscode-extension-samples/tree/master/comment-sample and modify the package.json to remove the icons from the delete or edit command
