refactor(app): unify pre-handshake guard to console.warn

Summary

_assertInitialized (#623) used console.error while _assertHandlerTiming (#629) and the AppBridge.replaceRequestHandler override used console.warn. This aligns all three on console.warn for the non-strict path; strict: true still throws for both App-side guards.

Also hoists the console.warn spy to beforeEach/afterEach in the pre-handshake guard test block — bun:test's spyOn was stacking when sequential tests spied on the same method, causing false positives once all three guards targeted console.warn.

Test plan

• bun test src/app-bridge.test.ts — 137/137 pass.
