Pass QueryContext to _retrieve_hybrid_results() so that expanded_queries computed by _analyze_query_context() are used in retrieval instead of being silently discarded. Supplemental semantic + keyword searches are fanned out in parallel for each expanded term; results are merged into the primary result set with content-hash deduplication so primary hits are never displaced.

Closes #4685
Summary
QueryContext.expanded_querieswas computed by query expansion but never passed to_retrieve_hybrid_results()— query expansion was a silent no-opexpanded_queriesinto the hybrid retrieval call so expanded query terms actually improve recall
