Duplicate Key Error for Address Updated Entity

This issue tracks an incident that caused ENSIndexer to crash.

• Incident 1: details (7 minutes duration).
• Incident 2: details (59 minutes duration).

@shrugs Noted the following:

summary: crashed because of a duplicate key for the address updated entity, but that's keyed by event log index so very confusing — ponder restarted and it progressed. it's feeling like rpc error with incorrect log index or something.

Subtasks:

• What is the root cause of this error? Can we reproduce it?
• Why did incident 2 cause 59 minutes of downtime? Why wasn't the restart faster?
