The example pages point to 404 pages

The example pages in the documentation - https://github.com/oauthjs/node-oauth2-server/blob/master/examples/password, https://github.com/oauthjs/node-oauth2-server/blob/master/examples/refresh-token and https://github.com/oauthjs/node-oauth2-server/blob/master/examples/authorization-code are pointing to 404 pages.

The example under model specification also has a 404.

Duplicate of #287. There are currently no examples for v3.

If you need examples for the version currently published on npm (oauth2-server@2.4.1) see this commit hash: b36a06b445ad0a676e6175d68a8bd0b2f3353dbf.

There are very similar examples in the express wrapper, I suppose also v2.x, but why not link there for the time being?
