Octicons missing from navigation tabs (repo & profile pages) when logged in

When logged into my account, the small icons next to navigation tabs are missing, both on repository pages (Code, Issues, Pull requests,
Actions, Wiki, Security, Insights) and on my profile page (Overview, Repositories, Projects, Packages, Stars).

What I've tried:

• Tested on multiple browsers: Chrome, Safari, Brave, and mobile browsers, same result on all of them, as long as I'm logged in
• Disabled all Feature Previews, no change
• Hard refresh and cleared cache, no change
• When logged out (or viewing the same pages in a private/incognito window
  without logging in), the icons display normally

This suggests the issue is tied to my account specifically rather than to any single browser or device.
Any help would be appreciated.
