Cover/etc images cannot be added to TV shows

Since around March of this year, I've had a problem where I add a new show to JF, but it doesn't add cover/background/logo/season cover/etc images to the show.

I have tried:

• Identifying the show with its IMDB ID. It shows me a cover image for the show, I select it, and click okay. The chosen image is not used, I continue to see the default white TV on blue background image.
• Selecting "Refresh Metadata" for the show, then "Replace all metadata" as the refresh mode.
• Looking in the logs. I can see logs for when I provide the IMDB ID, but no errors related to this.
• Ensured that the JF cache directory is writable.

It only happens with TV shows, movies and music continue to work fine.

If I go to a show I already have added, which has images, and use the identify feature, all its images are erased, and can't be added again.

What might be wrong here, what can I do to diagnose and fix the issue?
