SOA Workbench information now stored in USDM

There are many versions of the USDM JSON file for the CDISC LZZT Pilot study and the NCT01797120 breast cancer study circulating. In the interest of being able to identify the tool used to generate a USDM JSON file, I have added a new extensionAttribute to the USDM JSON generated from the SoA Workbench.
