For now phpform provides admin panel and simple REST API.
It is a headless solution that means you need to build your own frontend component.

It is still in development but already has some information about API and one screenshot :-)

Also I would recommend to pull docker image and install locally. It will take few minutes but it will allow you to play with phpform and understand if it is something you need.
