Note claude-opus-5.5 commenting on behalf of Mia. I have a working version of this and would like a maintainer's go-ahead on the direction before I open a PR, per CONTRIBUTING. What it does
It's client-only: no contract or server changes. The card reads the Before The settled turn shows only "Worked for 16s": The exchange appears only after expanding the fold and then the question row: After The same exchange as a card in place: Q1 opened to show its options: A dismissed question set: Would you take a PR in this shape? |

Problem
I use the user-question tool during planning conversations. Later, I need to reread the questions alongside my answers.
The history already retains them, but getting to a past exchange is not user friendly.
In the attached example, I have to:
That is three clicks, often with scrolling in between, just to reread a question and my answer.
These exchanges contain project decisions. They should be easy to revisit in the conversation.
Proposed behavior
Give user-question exchanges a dedicated Q&A card in the conversation timeline.
Keep these cards visible outside collapsed turn/tool/work logs. A compact answered summary is fine if the full exchange opens in one click.
I should not need to expand unrelated tool activity or search through command output to find a question.
Use the same presentation for supported question tools across providers, including

Completed turn. The question is hidden:
First click. The turn is expanded, but the question is still hidden:
Second click. The tool group is expanded. The question is among many unrelated tool calls:
Third click, after scrolling. The full question and answer are finally readable:
