Bump chart version + appVersion 0.11.0 -> 0.12.0 and regenerate the chart
README version badge. v0.12.0 headlines the pipeline stdout export feature
(kollect-pipeline collect --output -, NDJSON/YAML/JSON), plus HY-06 CI
concurrency hardening and launch-doc reconciliation.

Version is committed here (not bumped by CI); the vX.Y.Z tag on this commit
drives the tag-triggered Release workflow. CHANGELOG [0.12.0] and the roadmap
launch-truth line are updated post-tag (changelog-sync app + follow-up).
