docs(readme): action-setup-first README + single-row badges#50

The Marketplace listing renders this README. Put all hero badges on a single line (they were stacking vertically on GitHub's Marketplace markdown renderer, which keeps the line breaks), and pull a setup-first 'Gate your CI' section — now current for servers AND skills — to the top so an Action visitor sees how to set it up before the harness internals.
