Jira-to-plan integration

jira_to_plan tool: takes a Jira ticket ID, fetches description/acceptance criteria via Jira API, runs scope_work with that context. Bridges the gap between ticket assignment and first prompt. Needs Jira API token config in .preflight/config.yml.
