OIDC trusted publishing can't perform the first publish (the package must already exist before a trusted publisher can be configured). Authenticate the npm-publish job with the NPM_TOKEN secret (an Automation token that bypasses 2FA) so the bootstrap release publishes from CI, and pass --provenance to keep a signed build attestation (id-token: write is retained for that).
