This caused some CUE/BIN CD images to stop working
The previous fix for mixed unstored and stored pregaps (CUE file PREGAP and INDEX 00) broke standard unstored pregap handling. This adds back purely unstored pregap handling along with fixing a bug in the mixed pregap handling.
