Create a semantic Effectiveness Bar above the current structural Quality Bar so generated HTML is judged by whether it helps the user decide, act, continue, or reuse context — not only by required sections or keywords.
Scope: add docs/EFFECTIVENESS_BAR.md, reference it from make/pick/audit commands, keep QUALITY_BAR.md as structural floor, and validate with existing docs/command/golden checks.
No-scope: no deploy, secrets, VPS, DB, CSS redesign, Mermaid, charts, share/deploy, or broad refactor.
Acceptance: serious artifacts must state the job-to-be-done, work removed, evidence-to-decision link, first-screen decision/risk/next action, and copy/export output usefulness.
Rollback: revert the PR and return to QUALITY_BAR.md only.
