The Active Agents view already exposed session-level state, but users still had to mentally map flat agent rows back to worktree ownership. This change adds an explicit worktree layer for both ACTIVE AGENTS and CHANGES while keeping the existing watcher-driven refresh path and session-scoped actions intact.
