fix: update room_id description to reference active_conversation_id

Summary

• Updates the send_email tool's room_id parameter description to explicitly tell the AI to use active_conversation_id from the system prompt context
• Adds PR creation step to CLAUDE.md git workflow

The frontend chat's system prompt provides active_conversation_id but the send_email tool expects room_id. The AI didn't know to use one for the other.

Solution
