Finish the final integration pass after the parallel refactors by removing the remaining raw type-holder leaks from concrete module surfaces and tests. Add the missing HTMLFormElement root alias, normalize the remaining canvas, fetch, push, service worker, web socket, and shared worker surfaces, and keep the final diff free of new include directives.
